Jeff L. Horn

Jeff Horn 54 of Phoenix, AZ passed away peacefully surrounded by his family on November 6, 2013 at Eckstein Hospice Center, Scottsdale, Arizona. He was born February 18, 1959 in Appleton, and graduated from Appleton West High School in 1977.

Most of his life he lived in Phoenix where he enjoyed the outdoors, cooking, reading, sports and his love for The Green Bay Packers. He loved his friends and family with all his heart.

He is survived by his loving son Devin; his parents, Jo and Lou; brother Tim(Kris), and their daughters, Brittany and Lizzie; sister Becky (Paul) Batleyand their daughters, Madeline and Jacklyn; aunts: Char(Bill) Friede, Sue (Paul) Hansen; cousins: Barb Agness, Mike Weiland and Patti Weiland.

He was preceded in death by his grandparents, Earl and Helen Wichmann, Louis Sr. and Cora Horn; and brother, Jim Horn.
